Position Purpose:

The Clinical Intake Coordinator coordinates client referrals from all sources. Responsible for gathering necessary information from referral sources, promoting Agency services, and functioning as a clinical resource and customer service representative. Receives and processes client referrals as prescribed by the physician, and/or requested by the client/family, and in compliance with the state's Nursing Practice Act, any applicable licensure/certification requirements, and the organization's policies and procedures. Enters referrals into computerized database and forwards information in desired format to appropriate health providers and agency contacts. Evaluates referrals for client appropriateness for home health services. Receive and document medical orders, accept for services if appropriate and fully process (or delegate) referrals in an accurate and timely manner. Communicate information to involved parties according to Intake and Trinity Health at Home (THAH) guidelines. Obtains all payer information relevant to the referred client and assists with payer verification process. Responds to inquiries regarding homecare/hospice services. Develops and maintains positive relationships with all referral sources, internal and external customers, and fellow associates.
Position Highlights:


Ensures referrals are being handled in a customer friendly manner, accurate billing and demographic information is being entered for billing purposes and demographic and clinical information is being collected and transferred to the appropriate contacts.

Functions as a clinical resource coordinating complex cases. Ensures all clients (referral sources) needs are addressed.

Provides follow up on physician communication regarding client needs, home care services required or any other change in the initial order for care. Clarifies, verifies, and signs off on physician orders for initial services.

Responds to inquiries regarding agency services accurately identifying needs of clients and representing home care/hospice services in a positive, problem-solving way.

Ensures all insurance authorizations are being obtained, documented, and communicated within THAH policies and procedures.

Ensures all data entered the call center software, clinical application software, admission log/other reports as assigned, is accurately and timely.

Organizes data gathering systems to capture performance improvement for the intake department.

Assists in the identification, development, and implementation of new programs/initiatives as assigned by the director of customer service.

Initiates the referral process of all THAH clients from receipt of referral through the insurance verifications of all third-party payers to the completion of referral and communication to agency offices.

Converts client/physician inquiries into sales and displays excellent customer service skills. Works collaboratively with all phases of THAH marketing plan.

Assures that all required referral documentation is entered into the computer, the insurance source is verified, and the referral is communicated with the identified agency in a timely manner for scheduling the initial assessment nursing / therapy visit.

Communicates with all referral sources via multi-channel communication regarding potential client information, coordinates communication with the home care coordinators, home care agency staff, physician, other health care disciplines, client, and family, as appropriate.

Notifies all involved parties of changes in the client's condition and/or discharge status and the need for additional physician orders, obtaining and documenting verbal orders per policy, as appropriate.

Minimum
Qualifications:


Graduate of an approved Nursing education program.

Current Registered Nurse licensure in the state in which practicing.

Two years or more of Home Healthcare experience.

Must have current Driver's license and reliable transportation to and from work site.

Must be proficient in the use of McKesson's Horizon Home Care Software. Will accept applicable experience in lieu of specific Homecare Homebase experience.

Must be proficient in the use of computer office software.

Ability to consistently demonstrate a commitment to the mission and Organizational Code of Ethics and adhere to the Compliance Program.

Must possess excellent and clear oral & written communication skills.

1-3 years prior experience in a customer service, homecare intake or admissions role preferred.
